Recreation Activity Waiver

The recreation activity waiver for the EKU Rec Center (pool, indoor basketball, e-sports) is below. This is mostly for reference to review ahead of time. Hard copies will be pre-filled in and ready for you at registration to simply sign and go or decline permission if you wish.  If you are sending your child/student with someone other than their parent/guardian you may print and sent with them signed. 

This waiver can not be modified or changed please do not ask.
